Survey of mechanical ventilators in US acute care hospitals: a baseline for critical care surge capacity planning.

چکیده
The study by Rubinson et al reported in this issue of Disaster Medicine and Public Health Preparedness estimating the number and distribution of mechanical ventilators in the United States has been much anticipated and is important to emergency preparedness.1 It is surprising to many that this information was not previously known, but, in fact, no attempt to comprehensively count and characterize the ventilators in the United States had ever been attempted. Previous estimates of the total number differed by a factor of 2, from approximately 50 000 to approximately 100 000. The critical importance of this information became apparent following the terrorist attacks of September 11, 2001, and the 2001 anthrax attacks as federal planners began to consider the number of ventilators needed in the Strategic National Stockpile (SNS). Before one can make an informed judgment about how many additional ventilators may be needed in a disaster, an inventory of the existing supply is needed.

Mechanical ventilation in mass casualty scenarios.
Hospitals have long responded to acute multiple-casualty events, which lead to short-lived chaos followed by a more organized medical response and rapid health-system recovery. There has been increasing recognition, though, that events can occur that require a prolonged response for many critically ill patients. These disasters will require a well-coordinated regional and extra-regional medical...
متن کاملHospital disaster preparedness in Los Angeles County.
BACKGROUND There are no standardized measures of hospital disaster preparedness or hospital "surge capacity." OBJECTIVES To characterize disaster preparedness among a cohort of hospitals in Los Angeles County, focusing on practice variation, plan characteristics, and surge capacity. METHODS This was a descriptive, cross-sectional survey study, followed by on-site verification. Forty-five 9-...
متن کاملCritical care capacity in Canada: results of a national cross-sectional study
INTRODUCTION Intensive Care Units (ICUs) provide life-supporting treatment; however, resources are limited, so demand may exceed supply in the event of pandemics, environmental disasters, or in the context of an aging population. We hypothesized that comprehensive national data on ICU resources would permit a better understanding of regional differences in system capacity. METHODS After the 2...

متن کاملStrategies to improve pediatric disaster surge response: potential mortality reduction and tradeoffs.
OBJECTIVE To estimate the potential for disaster mortality reduction with two surge response strategies: 1) control distribution of disaster victims to avoid hospital overcrowding near the scene, and 2) expand capacity by altering standards of care to only "essential" interventions. DESIGN Quantitative model of hospital mortality. SETTING New York City pediatric intensive care unit and non-...
